Description[]
Doujun (道順) is a surgeon who conducts inhuman experiments on the prisoners of the Abandoned Dungeon. He is the last disciple of the master surgeon Dosaku, while the others have left him for Dogen.
Should Wolf accept to collaborate with him, Doujun will first request a samurai or a taro troop soldier to test the Rejuvenating Waters on and then the eyes of the Red-Eyed Carp of Mibu Village to finish the procedure.
Subjugated by the split personality of his master, he himself will imbibe the Waters and join the other misfortunate soul in his Red-Eyed madness.
Sekiro Side Story: Hanbei the Undying[]
Chapter 5[]
Since Doujun's Red-Eyed subject Yamahito, the mountain hermit that was previously captured in the Abandoned Dungeon, failed to defeat Hanbei, he makes a new plan with Takamine to lure the samurai to Ashina Castle by kidnapping Suzu.
The surgeon explains that Hanbei's superior type of immortality derives from the centipede in his body: this insect was imbibed in and adapted to the Rejuvenating Waters, giving him the power to regenerate the flesh of his host, no matter how severe his wounds are.
Chapter 6[]
In Ashina Castle Hanbei and Shoukichi find themselves face to face with Takamine, who admits he sent those "brigands" to kidnap villagers for Doujun's experiments, under the orders of Genichiro.
The samurai is victorious at last, but his friend was gravely injured during the fight and Doujun offers to save him, if Hanbei is willing to follow him into the Abandoned Dungeon.
Chapter 7[]
As the doctor is tending to Shoukichi, Hanbei is confronted yet again by Kodou Takamine, who Doujun healed from the Undying's fatal blow with his Rejuvenating Sediment.
On the brink of defeat, Hanbei is rescued by Shoukichi, but he's promptly cut down by Kodou.
Chapter 8[]
In the end, Hanbei manages to throw Kodou down a bottomless chasm, but when he finds a mind-deprived Suzu in a cell of the dungeon, he rather put her out of that misery at once.
As the undying samurai is about to leave the dungeon, Doujun sees that he set the entire place on fire, before experiencing a mental breakdown due to the bad karma he accumulated through his gruesome research.

- Underground Waterway: after acquiring Red Eyes, on the small island with three Ashina Soldiers.
Progression[]
Ashina Castle (Abandoned Dungeon Entrance)[]
Answer with "I'm a shinobi" to receive the Surgeon's Bloody Letter, which details the request of master Dosaku. He needs to have delivered to the dungeon either:
- A samurai: Jinzaemon Kumano, which means losing the Jinza's Jizo Statue.
- It won't be possible to send him, if Wolf has killed O'Rin of the Water prior to speaking with him or if Jinzaemon is at the Water Mill of Mibu Village.
- A member of the Taro Troop: Kotaro, which means losing either the Promissory Note or Taro Persimmon.
- Can't be sent if he's with Anayama the Peddler or if he has already received the White Pinwheel, only after giving him the Red and White Pinwheel.
After sending one of them, Doujun will reward Sekiro with Lump of Fat Wax [x3]. Exhaust all dialogues and reload the area to have Doujun move inside the subject's cell. After eavesdropping on him, reload the area once more and talk to him again: he'll give Wolf the Surgeon's Stained Letter, that details the task of killing the Red-Eyed Carp of Mibu Village for its eyes.
Ashina Depths (Water Mill)[]
Reach the entrance of the Wedding Cave to fight the Corrupted Monk and kill her for the Mibu Breathing Technique, which will now allow Wolf to dive underwater.
Ashina Depths (Mibu Village)[]
Dive in the pond to find the Red-Eyed Carp among some Man-Eating Fishes.
Ashina Castle (Abandoned Dungeon Entrance)[]
Hand the Red Carp Eyes to Doujun, who'll gift Sekiro Lump of Grave Wax [x2] in return. Exhaust all dialogues and reload the area to eavesdrop on him one last time while he's in the cell.
- While diving into the waters to reach the cell to exhaust his dialogues is possible even now, it's highly unrecommended as it'll trigger Doujun to move to the island alongside his patient.
Abandoned Dungeon (Underground Waterway)[]
The man sent to Doujun will now be found hostile and Red-Eyed on the small island where three Ashina Soldiers used to be, just past the Sculptor's Idol: he'll drop an unique Red Lump upon death.
Dive at the start of the waterway and turn left by the three Light Coin Purse and re-emerge at the end of the passage to find a grappling spot: after getting over the wall to drop inside the test subject's old cell and talk to Doujun: reload the area one more time and speak to him again to exhaust all his dialogues.

Behavior and Tactics[]
Perilous Grab: readies in a stance while moving slowly towards Wolf. When near, dashes forward quickly elbow first and if connected, wrestles him onto the ground before stomping.- Chasing Strikes: runs to the right and deliver a three hit combo.
- Double Round-house Kick: two spinning kicks.
- Exorcism: a slow upward palm strike.
- Leaping Kicks: runs forwards and does two spinning kicks, follows by an overhead kick.
- Praying Strikes: an elbow strike, follows by a palm strike and a jumping kick.
- Rock Throw: throws a rock at Wolf if he is too far away and out of reach.
Fights exactly like a Senpou Monk.
If faced after he gained Red-Eyes, he'll be much stronger and resistant, but the fire of the Flame Vent can be used to scare him momentarily.
Consuming a Gachiin's Sugar before diving in the waters of the Abandoned Dungeon makes it possible to perform a Backstab Deathblow on him and even turn the other Red-Eyed subject into an ally through the Puppeteer Ninjutsu against Doujun.

- Won't drop anything if killed prior to obtaining Red Eyes.
- Can't be harmed while infected with Dragonrot or crying for Dosaku in the cell.
- Can't be spirited-away with the Divine Abduction.
Trivia[]
- As Dosaku's Note implies, the master surgeon is most likely dead and Doujun is suffering from a split personality disorder.
- Given his attire and fighting style, Doujun probably used to be a Senpou Monk before relocating in the Abandoned Dungeon.
- It's safe to assume the Chained Ogre was the result of one of Doujun's experiments, especially given the description of the Second Prayer Necklace.
- It's also plausible that Doujun created the Rejuvenating Sediment that Genichiro consumed.
- Doujun is sometimes spelled Dojun: this inconsistency in translation also appears in Dosaku/Dousaku and Dogen/Dougen.

Unlike other characters, when offered the Dragonrot Pellet, Doujun will act skeptical, demanding to know what that medicine is. After being cured, he'll speak a little of Emma.

Some developer notes were left among the dialogues that present a different progression of Doujun's questline: it appears that instead of Kotaro, Wolf was supposed to choose between sending either Jinzaemon or Blackhat Badger (who has an unused entry to appear as one of Doujun's Test Subjects) to the Abandoned Dungeon and to bring back his liver to Doujun for him to finalize the experiment.
